Because forms are in triplicate, they are not available for download from our site. You may obtain forms from your Service Unit Coordinator or Membership Development Manager. If they are unavailable, please contact the council office at 312/416-2500 x258 or abowden@girlscouts-chicago.org. Don't forget, it is important to let your Service Unit Coordinator know about all troop trips, whether or not extra insurance is needed.
